Patient-Controlled Health Memory
Projection: Note automation and medical evidence products do not yet give individuals a portable longitudinal account of their care. A permissioned summary could travel between agents while raw records remain distributed. Reuse across institutions, with patient-controlled access and no new central data lake, is the necessary demonstration.
Why this idea is here
What the evidence establishes.
The cited captures document clinical documentation and evidence-tool adoption. They do not establish Patient-Controlled Health Memory as a deployed mechanism. The adoption test is whether patients reuse permissioned longitudinal summaries without centralizing every raw record.
